Electrical Service Application

Type of Applications
  •           A. Proof of Identity

                          1. Personal

                                       a.) Valid ID (government-issued IDs)

                                       b.) If represented, Special Power of Attorney

                          2. Or Partnerships, Corporations, and Non-government Organizations

                                       a.) Board Resolution or Secretary's Certificate or Certificate of Appointment

                                             or Company ID indicating the position (President, Chairman, Branch

                                             Manager) - for the Corporation

                                       b.) Valid ID of the appointed signatory (government-issued IDs),

                                       c.) BIR Certificate of Registration (BIR form 2303)

              B. Proof of Ownership of the Property

                          1. Land Title

                                       a.) If named to the old owner, Deed of Sale

                                       b.) Or if named to a deceased kin, Affidavit of Heirship

                          2. For properties not owned

                                       a.) Lease Contract if rented

                                                   (1) Affidavit of Undertaking of Jointly and Solidary Obligations

                                       b.) Or, notarized consent from the owner

              C. Electrical Permit / Certificate of Final Electrical Inspection

                           1. Secure from the local government engineering/building office

              D. Bill Deposit

                           1. Equivalent to a 1-month projected bill based on the applied load in Watts

              E. Special Deposit to Advance the Project Cost - For applications beyond the Standard

                  Connection Facilities (SCF) of the Distribution Utility

                           1. Residential - SCF for residential end-users shall be within five hundred (500)

                               meters from Distribution Utility’s existing secondary low voltage lines and a

                               transformer size requirement of up to 1 x 50 kVA for single-phase or 3x50 kVA.

                           2. Or Non Residential - For non-residential End-users, the SCF shall be within thirty

                               (30) meters from the distribution line and a transformer size requirement of

                               lower than 3 x 100 kVA.

                           3. Temporary for Construction applications that will need a distribution transformer

  •       A. Proof of Identity

          B. Bill Deposit - Equivalent to the 12 months average bill before service was disconnected.

    (For Accounts with over 1 year of Inactivity or with meter removed more than a year)

          C. Proof of Ownership of the Property

          D. Electrical Permit for Reconnection/ Certificate of Final Electrical Inspection

    1. Proof of Identity

    2. Proof of Ownership of the Property - succession of ownership

    3. Bill Deposit - Equivalent to the 12 months average bill of the old account.

  • Senior Citizen Discount Requirements

    1. Senior Citizen ID

    2. Senior Citizen Application Form

    Note:

    • Account must be under Senior citizen's name

    • Applicable for residential classification only

    • The 5% discount will automatically be deducted to their monthly bill if their consumption is not more than 100kWh

    • Discount shall apply per household regardless of the number of senior citizens residing therein will be applied annually

  • Lifeline Subsidy Discount Requirements


    • Duly accomplished lifeline subsidy application form

    • Most-recent electricity bill for the service being applied for

    • Valid government-issued ID containing the signature and address of the consumer

    • Certification from the local SWDO issued within six (6) months - (For Non 4Ps Lifeliner/Renewal)


    Other Supporting Documents


    • If electric service is not registered under the name of the applicant: Proof of Residence

    • If the application is filed through a representative: Letter of Authorization

    • Valid government-issued ID (with signature) of the representative

    • In case of transfer of residence:  Re-issued Certification from the local SWDO

Step 1

Secure an electrical permit from the Office of the City Engineer if the installation is within Cotabato City limits or from the Office of the District or Municipal Engineer if the installation is within D.O.S or Sultan Kudarat respectively.

Step 2

Accredited electrician to present the electrical permit to the Customer Service Staff, and you will be issued a meter base by the Service Section, free of charge.

Step 3

Have your accredited electrician install the meter base at your service entrance, facing the street and accessible for meter reading and maintenance.

Step 4

Secure a Certificate of Electrical Inspection (CEI) from the Office of the City Engineer if the installation is within Cotabato City, or from the Office of the District or Municipal Engineer if it is outside the city. Let your accredited electrician request for inspection after having the CEI.

Step 5

Our inspector will then visit your residence or establishment to make sure that your service entrance conforms to Government regulations and Cotabato Light standards.

 

Our inspector will advise you of any adjustments that your installation may require. You may also call our Service Section (Tel no. 520-2572) for the result of the inspection.

Step 6

When your application is approved, the applicant must pay the initial bill deposit equivalent to  one month's estimated consumption and then sign the contract for electric service.

 

This completes your application. The Cotabato Light Service Section will then schedule the installation of your electric meter within 3 days.

How to Apply

This completes your application. The Cotabato Light Metering Services will then schedule the installation of your electric meter within 3 days.
